Overview

We are seeking a dedicated and experienced Case Manager to join our TEAM!

 

NOTE: This is NOT a remote position

It is 100% in office.

The Case Manager is responsible for providing case management services to a variety of populations, following applicable State and Federal, and accreditation guidelines.

 

Responsibilities

 

The Case Manager is responsible for providing case management services to a variety of populations, following applicable State and Federal, and accreditation guidelines.

  • Maintains contact with resources including schools, families, legal, employment, and referral sources
  • Completes documentation and communicates with schools, discharge planning, FMLA/STD, courts, and return to work letters
  • Communicates information provided by patients or other sources to the interdisciplinary team.

Qualifications

 

COVID-19 Vaccination Requirement
Advanced Recovery Systems requires all employees who work in any facility or office to be fully vaccinated against COVID-19 (including any booster shots if required), where allowable under the law, unless they are approved for a reasonable accommodation based on disability, medical condition, pregnancy, or religious belief that prevents them from being vaccinated. 

    • Bachelors Degree in health-related field preferred.
    • Minimum two years preferred experience in the medical and/or behavioral healthcare field
